Make Up Lesson / Class Policy*

I understand that makeup lessons or classes are only eligible for active members. All makeup lessons or classes must be taken prior to the student’s last lesson day.

Invoicing Policy*

I agree to set up automatic payments through the invoicing process. I understand that the monthy payments are due on day 1 of each month.

Membership Agreement*

I agree to submit a lesson / class termination through the form on the website when I decide to take a break from lessons. I also agree that if I submit this form on or after the 16th day of the month, I will be charged a full month of tuition on the following month. Students will be able to still take the lessons that they paid for during that time period.

Marchwood Music likes to demonstrate lessons and also highlight student milestones. With your permission, we would like to occasionally share a video or photograph as a teaching opportunity. I hereby give consent to Marchwood Music to photograph, videotape, or otherwise digitally record and use images and/or sound recordings of myself or my child or children (if applicable) to use for instructional or promotional use on social media.
